Yes I would like to dispute the notice and also would like to make changes
to remove possibly infringing code.

Yes, my repository contains HTML code from Optum's website because my app
relies on other pages in order to work and is present solely for testing
purposes. Also, my work is licensed under MIT License so their claim that
my repository isn't licensed is false. The rest of the source code is
original work done by me.
