This reverts commit r327566, it breaks
test/ExecutionEngine/OrcMCJIT/test-global-ctors.ll.
The test doesn't crash with a stack trace, unfortunately. It merely
returns 1 as the exit code.
ASan didn't produce a report, and I reproduced this on my Linux machine
and Windows box.

Layer implementations typically mutate module state, and this is better
reflected by having layers own the Module they are operating on.

than a shared ObjectFile/MemoryBuffer pair.
There's no need to pre-parse the buffer into an ObjectFile before passing it
down to the linking layer, and moving the parsing into the linking layer allows
us remove the parsing code at each call site.

This patch updates the ORC layers and utilities to return and propagate
llvm::Errors where appropriate. This is necessary to allow ORC to safely handle
error cases in cross-process and remote JITing.

Revert "[ORC] Remove redundant semicolons from DEFINE_SIMPLE_CONVERSION_FUNCTIONS uses."
Revert "[ORC] Move ORC IR layer interface from addModuleSet to addModule and fix the module type as std::shared_ptr<Module>."
They broke ExecutionEngine/OrcMCJIT/test-global-ctors.ll on linux.

clang-format (https://reviews.llvm.org/D33932) to keep primary headers
at the top and handle new utility headers like 'gmock' consistently with
other utility headers.
No other change was made. I did no manual edits, all of this is
clang-format.
This should allow other changes to have more clear and focused diffs,
and is especially motivated by moving some headers into more focused
libraries.

At the same time, fixes InstructionsTest::CastInst unittest: yes
you can leave the IR in an invalid state and exit when you don't
destroy the context (like the global one), no longer now.

Type specific declarations have been moved to Type.h and error handling
routines have been moved to ErrorHandling.h. Both are included in Core.h
so nothing should change for projects directly including the headers,
but transitive dependencies may be affected.

Bypassing LLVM for this has a number of benefits:
1) Laziness support becomes asm-syntax agnostic (previously lazy jitting didn't
work on Windows as the resolver block was in Darwin asm).
2) For cross-process JITs, it allows resolver blocks and trampolines to be
emitted directly in the target process, reducing cross process traffic.
3) It should be marginally faster.
